Personify Care supports Monash Health to optimise planned surgery waitlist with digital patient pathways

To reduce patient wait times and improve patient access to care, Personify Care is supporting Monash Health to optimise planned surgery waitlists by converting their model of care into a digital pathway.

The technology helps identify low risk patients eligible for direct access to their procedure and reduces the need for an in-person pre-admission assessment.

A Personify Care digital pathway was implemented for Flexible Cystoscopy Procedures to simultaneously reduce the burden on staff, provide early visibility of patients clinically eligible, and provide patients with digital confirmation of their perioperative instructions, and procedure date and time.

Patients receive a secure link from Monash Health to access their pathway that’s accessible on any device – computer, tablet, or phone. This daily checklist covers what they need to do, information they need to know, and clinical assessments they need to complete.

Personify Care’s digital patient pathways are now being deployed to support planned surgery waitlist optimisation including direct access endoscopy pathways, with full integration with Monash Health’s digital record and the iPM Patient Administration System.
