More than just another patient portal

We help staff at hospitals and surgical centres map their pre-admission and post-op patient pathways so that nursing staff can seamlessly screen pre-admission risks and track post-discharge recovery without manually processing paper forms or making time-consuming phone calls.

We offer a mobile platform that can be configured by staff to track patient pre-admission and / or post-discharge pathways and produces patient response rates of over 80%.

Patients get a secure checklist on their mobile so that they are better prepared before surgery and have a better recovery post-discharge. Nursing staff get visibility via a dashboard of the patients that require follow-up based on their progress in the previous 24 hours against existing clinical criteria.

How do I get access to Personify Care? How much does it cost?
If you would like to learn more about the product, you can book a demo with us here. After the demo, we can create an account for you and setup a trial so you can start using it within your organisation.

Personify Care is offered as a monthly subscription per clinical user, based on patient volume and the complexity of the protocols being delivered.

Can Personify Care be used with my EHR or Patient Management System?
Yes. Personify Care is designed to capture patient outcomes in between clinical interactions, augmenting the patient’s medical record. The Personify Care platform is designed to integrate with EHRs and patient management systems. We don’t believe in creating more information silos. The Personify Care platform provides a well-documented API for your systems to communicate with, and supports HL7 (and other data exchange standards) for complete interoperability and ensuring no double entry of information.

Personify Care has also been successfully rolled out in clinical settings where the paper is still used for medical records. A patient summary can be printed as a pdf summary to be stored in the patient’s existing records. Personify Care does not require integration. Getting started does not require an IT project, installation of any software or internal IT support.

How many patients should we have to use Personify Care?
Personify Care scales from a single specialist practice with as little as 10 patients per month to large hospital groups with over 100,000 patients across their organisation. Typically, the platform is introduced within a specialist practice or nursing group for a particular specialty and then expanded across the organisation.
Can I use Personify Care for my research project or clinical trials?
Yes. In fact Personify Care allows clinicians to reduce the costs of monitoring patient risks and measuring patient outcomes when conducting clinical trials. The platform also allows clinicians to evaluate the efficacy of their protocols by comparing the outcomes against a that of a control group. Personify Care is also seeking to collaborate with research and clinical organisations to evaluate the health and economic impacts of detecting patient risks early
Does Personify Care support standardised Health Outcomes Measurement?
Yes. Personify Care has implemented a number of the ICHOM standards published at ichom.org. Personify Care is also becoming an official ICHOM Technology Affiliate to allow clinicians to measure their outcomes as part of standard post-discharge care.
What about older patient populations?
Personify Care is used by patients up to the age of 81 (so far). Patients value getting a regular text message from their nurse and the patient checklist has been specifically designed with at-risk populations in mind. Patients don’t need any special technical knowledge – if they can send and receive a message then they can use Personify Care. Smart phones are also not a requirement with some patients tracking their recovery on their home desktop computer.

Our learning system has been specifically designed to optimise for response rates based on user behaviour. The more patients use our platform the better the system will get at reaching broader populations across more complex protocols.
